Documentation required for each participant
Support Plan
The Participant agrees that Made To Help Pty Ltd will bill a total of one hours and 30 minutes minimum to create, finalize, and review a standard Support Plan. The rate for these three hours will be charged in accordance with the relevant NDIS Price Guide support item and hourly rate applicable on the date the services are provided.
Emergency Plan
The Participant agrees that Made To Help Pty Ltd will bill a total of one hours and 30 minutes minimum to create, finalize, and review n Emergency Plan (as referenced under “Emergency and Disaster Planning”). The rate for these three hours will also be charged in accordance with the relevant NDIS Price Guide support item and hourly rate applicable on the date the services are provided.
Vulnerability Assessment
The Participant agrees that Made To Help Pty Ltd will bill a total of one hours and 30 minutes minimum to create, finalize, and review a standard Vulnerability. The rate for these hours will be charged in accordance with the relevant NDIS Price Guide support item and hourly rate applicable on the date the services are provided.
These charges will appear on invoices separate from any other supports, and payment terms follow those outlined in the “Payments” and “Fees for Supports” sections of this Agreement.

Why Support Plans Are Required
Support plans outline how a participant’s needs and goals will be met. They are essential for:
Person-Centered Care
Ensures the participant’s goals, preferences, and strengths are central to how support is delivered.
Required under the NDIS Practice Standards – Core Module: Person-Centred Supports.
Consistency and Continuity of Support
Allows all workers and providers to follow a consistent, agreed approach.
Helps with handovers when staff change.
Risk Management
Helps identify risks (e.g. medical, behavioral, or environmental) and set out proactive strategies.
Aligns with the NDIS Practice Standards – Risk Management requirements.
Evidence of Compliance and Quality
Registered providers are expected to document service delivery plans to meet audit standards.
Auditors review support plans to ensure they align with best practice and participant needs.
Why Emergency Plans Are Required
Emergency or contingency plans are vital to prepare for unexpected events that may disrupt the participant’s care or compromise safety:
Protect Participant Safety
Ensures the participant has a documented response in case of a medical emergency, natural disaster, carer absence, or service failure.
This is particularly important for participants who are vulnerable or have limited informal support.
Service Continuity
Outlines backup arrangements if the primary worker is unavailable.
This is part of continuity of support obligations under the NDIS Practice Standards – Provider Governance and Operational Management.
Obligation Under High-Risk Modules
Participants receiving high-intensity daily personal activities, complex health supports, or behaviour support may require an emergency response plan as part of their service plan.
NDIS Quality and Safeguards Commission Expectations
Providers must demonstrate they are prepared for risks, including environmental, health, or staffing emergencies.
Emergency planning is considered evidence of duty of care.
Why Vulnerability Assessment Are Required
- NDIS Code of Conduct: Providers and workers are required to deliver supports in a safe and competent manner, acting with integrity and promptly addressing concerns that may impact the quality and safety of supports provided to people with disability.
- Conflict of Interest Guidelines: The NDIS Practice Standards advise providers to implement strategies that prevent conflicts of interest, which can include ensuring that no single worker holds excessive influence over a participant’s care.
- Operational Guidelines on Informal Supports: The NDIA recognizes the importance of sustaining informal support networks and advises against arrangements where a participant relies solely on one individual, as this can compromise the participant’s wellbeing and the carer’s capacity to provide ongoing support.
